Required Credentials

For preschool:

  • Registration form

  • Photocopy of Birth Certificate

  • 2pcs 1x1 recent colored ID pictures

 

For Grade school:

  • Registration form

  • Report card from previous school attended

  • Good Moral Certificate duly signed by the school head

  • Photocopy of Birth Certificate

  • 2pcs 1x1 recent colored ID picture

 

For Special Education

  • Registration form

  • Latest Report from Developmental Pedia

  • Present Level of Performance from previous school (For transferee)

  • Photocopy of Birth Certificate

  • 2pcs 1x1 recent colored ID pictures

For Online Registration

1. Download and fill up the Registration Form provided above.

2. Email it at tots.work@yahoo.com

3. Upon Receiving your Registration Form, You may pay reservation or enrollment

     fees thru online bank transfer or deposit at PSBank Acct. no. 219-112-00006-4             under Account Name Tots @ Work Developmental Center for Young Learners, Inc.
